Insurance Policies:

I am an “Out-of Network” provider only. Payment will be collected in full at the time of service. If you are trying to use your insurance to assist in payment of services I can provide an itemized receipt for you to submit to your insurance company. Depending on the provisions of your insurance plan massage visits may go towards your deductible, or if there is coverage you will be directly reimbursed the determined amount by your insurance company.

Please Note:
  • It is your responsibility to know your plans coverage & any requirements associated (ie: prescription, or preauthorization). All requirements and interactions with your insurance company will be your sole responsibility.
  • I do not do pre-authorizations
  • HSA (Health Savings Account) cards are gladly accepted for payment.
MVA's $150

I do not bill for Motor Vehicle Accidents. Payment for each visit will be collected at the time of service and a receipt will be provided for you to personally submit to your claims agent. Any documentation requested by a lawyer or claims agent will be provided upon request, know that an additional administration fee may be applied for this request.

Cancellation Policy

Please allow 24 hours notice for all cancellations. Any missed or non-cancelled appointments with less than 24hrs notice will be billed at the full rate to you the client. Note that your insurance company can not be billed for missed appointments, it will be your responsibility.
